Job summary

Atrio Home Care is seeking a compassionate Physical Therapist to provide in-home visits in Grand Rapids or Holland, MI. We offer both full-time and part-time schedules with a 8 AM–5 PM day shift, plus a $5,000 sign-on bonus and a pay-for-activity model with weekend/holiday differentials.

Ideal candidates hold a Master's in Physical Therapy, a Michigan license, and at least 1 year of PT/home health experience, with geriatric experience highly valued.

• Master's degree in Physical Therapy (required).
• Michigan PT license (required).
• 1+ year of PT experience (home health preferred).
• Geriatric experience valued.

Responsibilities

• Provide one-on-one in-home physical therapy services.
• Conduct evaluations and develop individualized treatment plans.
• Collaborate with physicians and interdisciplinary team members.
• Help clients regain mobility, strength, and independence.
• Document progress and support discharge planning.
• Educate clients, caregivers and families on therapy goals and techniques.

• Weekend Rotation: once every 4 weeks, no on-call
