Job summary

Total Care Therapy is hiring a Physical Therapist in Grand Rapids, MI for a full-time role. You will assess residents, design individualized treatment plans, and deliver PT interventions to improve mobility, reduce pain, and maintain independence in senior living settings.

As part of a therapist-owned, supportive organization, you will collaborate with an interdisciplinary team, supervise PTAs, and document care in the EMR while advancing your clinical skills and professional growth.

Qualifications

• Licensed as a Physical Therapist in Michigan (mandatory).
• CPR/BLS certification required.
• Valid driver's license with reliable transportation.
• Experience in geriatrics, home health, assisted living, or skilled nursing preferred.

Responsibilities

• Evaluate residents and develop individualized plans of care.
• Provide skilled PT treatments: exercise, gait, balance, transfers, mobility.
• Reassess progress and modify plans as needed.
• Supervise PTAs per state regulations.
• Document care in the EMR accurately and timely.
• Educate residents, caregivers, and staff on treatment plans and safety.

About the Role We are seeking a compassionate and motivated Physical Therapist to join our growing team. In this role, you will evaluate, develop, and implement individualized treatment plans that help residents improve mobility, restore function, reduce pain, and maintain independence.

Responsibilities

• Evaluate residents and develop individualized plans of care.
• Provide skilled physical therapy treatments, including therapeutic exercise, gait training, balance training, transfer training, and functional mobility interventions.
• Reassess patient progress and modify treatment plans as appropriate.
• Supervise Physical Therapist Assistants (PTAs) in accordance with state regulations.
• Complete timely and accurate documentation using the electronic medical record (EMR).
• Educate residents, caregivers, and facility staff on treatment plans, home exercise programs, fall prevention, and safe mobility techniques.
• Collaborate with interdisciplinary team members to provide coordinated, patient-centered care.
• Maintain a productive clinical caseload while delivering exceptional therapy services.

Qualifications

• Current State Physical Therapist (PT) License
• Current CPR/BLS Certification
• Valid driver's license, reliable transportation, and auto insurance
• Experience in geriatrics, home health, assisted living, or skilled nursing preferred but not required
