Best Home Health Providers is seeking a Per Diem Physical Therapist in California to deliver skilled home health services in patients’ homes. You will evaluate patients, create customized treatment plans, and work with physicians to implement care plans.

The role requires a CA PT license, CPR certification, and at least 2 years of PT experience. A reliable vehicle and flexible schedule are essential, with potential conversion to full-time based on performance.

Qualifications

• Bachelor’s or Master’s degree in Physical Therapy from an accredited program.
• Active California Physical Therapy license.
• Minimum 2 years of PT experience; home health background is a plus.
• Strong communication skills and the ability to connect with patients and families.
• Current CPR certification with AHA.
• Valid driver’s license and reliable, insured vehicle.

Responsabilités

• Evaluate patients’ functional status, mobility, muscle strength, gait, and ROM to develop individualized treatment plans.
• Deliver physical therapy services in accordance with the physician’s plan of care and California’s Physical Therapy Practice Act.
• Educate patients and their families or caregivers on therapeutic exercises, equipment use, and recovery goals.
• Collaborate with physicians and the care team—reporting changes in patient condition and participating in case conferences.
• Supervise Physical Therapy Assistants and oversee Home Health Aide care bi-weekly when PT is the sole skilled service.
• Manage documentation including clinical progress summaries and discharge planning.
• Identify needs for additional home health services and coordinate referrals as appropriate.
• Perform other duties as required for patient services.

Please note that this role is per diem with a rate of $160 - $180 per visit.
