• Maintain accurate documentation of evaluations, plans, progress, and discharge summaries.
• Communicate plans, progress, and discharge recommendations with care teams and families.
• Educate patients and families on therapy, home programs, and injury prevention.
• Support motivation and therapy compliance.
• Collaborate with physicians, nurses, and other therapists.
• Participate in team meetings and discharge planning.
• Follow regulatory policies, documentation standards, safety, and infection control.
• Participate in quality improvement initiatives.
• Perform other duties as assigned.

Job description

• Conduct comprehensive evaluations of patients' physical conditions, functional limitations, and mobility
• Develop individualized treatment plans based on clinical assessments and patient goals
• Utilize therapeutic techniques including exercises, manual therapy, ultrasound, electrical stimulation, and heat/cold therapy
• Maintain accurate and timely documentation of evaluations, treatment plans, progress notes, and discharge summaries
• Communicate treatment plans, progress, and discharge recommendations to healthcare professionals, patients, and families
• Educate patients and families on therapy, home exercise programs, and injury prevention
• Support and encourage patients to improve motivation and therapy compliance
• Collaborate with physicians, nurses, occupational therapists, and speech therapists
• Participate in team meetings, case conferences, and discharge planning
• Follow regulatory and facility policies, documentation standards, safety protocols, and infection control procedures
• Participate in quality improvement initiatives
• Perform other duties as assigned

Requirements

• Graduate of a physical therapy curriculum accredited by the American Physical Therapy Association (APTA) or the Commission on Accreditation in Physical Therapy (CAPT)
• Hold a valid State Physical Therapy License
