Job summary

BrightStar Care is seeking a licensed Physical Therapist (PT) for part-time/per diem in Gardner, MA. You will conduct in-home evaluations, develop care plans, and deliver interventions to improve strength, balance, mobility and safety in home settings.

Requirements include MA PT license, DPT or MPT, CPR certification, and a reliable vehicle. New grads welcome; mileage reimbursement available, with flexible scheduling to fit your availability.

Qualifications

• Active MA PT license.
• DPT or MPT from an accredited program.
• CPR certified.
• 1+ year of experience preferred (new grads welcome).
• Reliable vehicle & valid driver’s license.

Responsibilities

• Perform in-home PT evaluations and create personalized care plans.
• Provide skilled PT interventions to enhance strength, balance, mobility, and safety.
• Educate clients and families on exercises and home safety.
• Collaborate with a multidisciplinary care team.
• Document care using mobile-friendly EMR.
