Job summary

All Care and Comcare Home Health is seeking a compassionate Field Physical Therapist to provide skilled therapy in patients' homes across our service area. You will perform SOC/ROC/recert and DC evaluations, tailor plans, and work with an interdisciplinary team to meet personalized goals.

We value strong communication, documentation, and the ability to travel to patient homes. A CA PT license, CPR, and valid CA driver’s license are required. Competitive salary and benefits are offered.

Qualifications

• Active California Physical Therapy license is required.
• Graduate of an accredited Physical Therapy program.
• Current CPR certification is required.

Responsibilities

• Provide comprehensive physical therapy services per physician orders.
• Perform SOC, ROC, recertifications and discharges evaluations.
• Assess patient skilled needs, plan interventions, evaluate outcomes, and adjust care.
• Collaborate with the interdisciplinary team to set and monitor patient goals.
• Educate patients, families, and caregivers on home exercise programs.

Position

Field Physical Therapist

Key Responsibilities

• Provide comprehensive, skilled physical therapy services and interventions in accordance with physician orders.
• Perform start of care (SOC) evaluations, resumption of care (ROC), recertifications (Recert), and discharges (DC).
• Assess patient skilled needs, plan interventions, evaluate outcomes, and modify care to meet physical therapy goals.
• Collaborate with the interdisciplinary team to develop and evaluate individualized patient goals.
• Educate, instruct, and train patients, families, and caregivers on physical therapy programs and home exercise regimens.

Qualifications

• Active license/registration with the Physical Therapy Board of California.
• Graduate of an accredited Physical Therapy program.
• Strong documentation and organizational skills.
• Excellent communication and interpersonal abilities.
• Willingness to travel to patient homes for evaluations and treatment.
• Flexibility regarding work locations throughout our service area.
