Job summary

Trilogy Home Healthcare in Gainesville, FL is hiring a Physical Therapist (PT) for home health visits on a PRN/per diem basis. You will deliver one-on-one PT services, help patients improve mobility and balance, and assess home safety and mobility needs.

Requirements include a graduate PT degree, a current FL PT license, and a valid FL driver’s license with insurance and clean MVR. 1+ year experience preferred, with home health experience a plus.

Qualifications

• Graduate of an accredited PT program.
• Current Florida PT license.
• Valid Florida driver's license, active vehicle insurance, and clear MVR.
• 1+ year of experience required, Home Health experience preferred.
• Sound judgement and a passion for patient care and outcomes.
