Responsibilities

• Conduct PT assessments of referred patients to evaluate medical conditions, functional capabilities, limitations, and rehabilitation potential.
• Provide direct patient care to facilitate rehabilitation.
• Maintain thorough patient documentation in PCC EMR.
• Collaborate with interdisciplinary team to support optimal patient outcomes.

Qualifications:
