Job summary

Therapeutic Home Healthcare is seeking a full-time Physical Therapist in Fresno, CA to conduct comprehensive client assessments, develop treatment plans with physician input, and implement interdisciplinary care plans. The role includes supervising PT staff, coordinating outpatient services, and maintaining detailed clinical records.

The position offers a $10,000 sign-on bonus paid quarterly in the first year, along with benefits such as 401k, dental, vision and health insurance.

Responsibilities

• Provides initial and ongoing comprehensive assessments including OASIS assessments.
• Establishes a treatment plan with physician consultation and revisions as needed.
• Assists in development/implementation of the interdisciplinary client care plan for Physical Therapy.
• Evaluates patients using diagnostic and prognostic tests.
• Treats clients to relieve pain and restore function using physical therapies.
• Instructs and counsels family and client in the physical therapy program.
• Instructs client in care/use of mobility aids and orthotics.
• Provides necessary equipment to carry out the plan.
• Coordinates for outpatient services when needed.
• Records evaluation data, treatments, and responses in a timely manner.
• Prepares assessments and periodic evaluations.
• Supervises Physical Therapy Assistants and paraprofessionals monthly.
• Records and reports patient status to physician and adjusts orders as needed.
• Communicates with physician every 30 days or as changes occur.
• Collaborates with other staff and instructs them as appropriate.
• Writes progress notes the day of visit and files within 72 hours.
• Participates in case conferences and staff meetings.
• Supervises home health aide when PT is sole discipline involved.
• Involves team in discharge planning and completes discharge summaries.

Responsibilities

1. Provides initial and ongoing comprehensive assessments of the client’s needs, including Outcome and Assessment Information Set (OASIS) assessments.
2. Establishes a treatment plan in consultation with the physician and revises it with the approval of the physician as necessary.
3. Assists in development and implementation of the interdisciplinary client care plan as it pertains to Physical Therapy.
4. Assists the physician in evaluating patients by applying diagnostic and prognostic muscle, nerve, joint and functional ability tests.
5. Treats clients to relieve pain, develop or restore functional and maintain maximum performance, using physical means, such as exercise, massage, heat, water, light and electricity.
6. Consults and familiarizes staff regarding the established plan.
7. Teaches, supervises, and counsels the family and client in the physical therapy program.
8. Instructs client in care and use of wheelchairs, braces, crutches, canes, and prosthetic and orthotic devices.
9. Provides the equipment required to carry out the plan.
10. Arranges for out-patient service for procedures which cannot be given at home.
11. Records on a timely basis all evaluation data, treatments, and client’s response to therapy intervention. Also records quality assurance data.
12. Prepares an assessment of the client’s condition and periodic evaluations and summaries.
13. Supervises Physical Therapy Assistants and paraprofessionals at least once monthly.
14. Records and reports to the physician the client’s reaction to treatment and/or any changes in the client’s condition.
15. Communicates with the physician at least every 30 days or whenever changes occur regarding the client’s status and obtains additional orders, if necessary.
16. Communicates with other team members (including Home Health Aides) and, when appropriate, instructs them in certain phases of physical therapy that they may use while working with the client.
17. Writes clinical/progress notes on the date of the visit and incorporates same in the client’s record within 72 hours.
18. Participates in case conferences, physical therapy staff meetings, agency activities and inservice programs as necessary.
19. Supervises the home health aide every two weeks when physical therapy is the only professional discipline involved in the client’s care and supervises the Physical Therapy Assistant at least monthly.
20. Participates with staff, clients, and physicians in discharge planning activities and completes a Physical Therapy Discharge Summary.
