Job summary

Trinity Healthcare Talent Services seeks a licensed Physical Therapist for our SNF community. You’ll evaluate patients, design tailored rehabilitation plans, and guide hands-on therapy to improve strength, mobility, and range of motion.

Responsibilities include educating patients and families, documenting progress, coordinating with doctors and nurses, and ensuring ethical, patient-centered care. A 1–2 year background in a similar setting is preferred; Mon-Fri with weekend rotation; hourly pay

Qualifications

• Bachelor's, Master's, or Doctorate in Physical Therapy from an accredited program.
• Current state license to practice as a Physical Therapist.
• Minimum 1–2 years of experience in a similar setting preferred.
• Strong communication and interpersonal skills.
• Knowledge of and adherence to legal and ethical standards in patient care.

Position Type: Full-Time, Monday-Friday with weekend rotation

Facility: SNF Community offering post-hospital, short-term, long-term, and respite care

Number of Beds: 181 SNF beds

Job Description:

We are seeking a highly skilled and compassionate Physical Therapist to join our team at our SNF community. As a Physical Therapist, you will work with a diverse patient population, providing rehabilitation services to individuals with a variety of conditions and injuries.

Responsibilities:

• Conduct initial evaluations and assessments to determine patients' needs and develop individualized treatment plans
• Provide hands-on therapy and exercises to improve patients' strength, mobility, and range of motion
• Educate patients and their families on exercises and techniques to continue their rehabilitation at home
• Monitor and document patients' progress and adjust treatment plans as needed

• Communicate effectively with patients, families, and caregivers to provide support and guidance throughout the rehabilitation process

Requirements:
