Job Details

• Position Type: Full Time
• Description: The Physical Therapist (PT) conducts assessments and reassessments of rehabilitation needs and potential. The PT plans, implements, and supervises therapeutic programs including exercises, gait training, range of motion testing, maintenance therapies, heat treatments, baths, and other activities typically performed by a physical therapist.

Job Duties/Knowledge

• Conduct assessments and reassessments of patient functionality using diagnostic and prognostic procedures.
• Develop and supervise individualized therapeutic programs to restore or improve physical functions.
• Coordinate with the Rehab Director and other professionals to integrate therapy into the overall care plan.
• Guide and instruct patients in techniques and exercises to enhance their recovery.
• Monitor and document patient responses and condition changes.
• Educate patients' families and agency personnel on physical therapy activities.
• Participate in case conferences and quality assurance activities.
• Supervise PTA treating evaluated patients and be available for consultations.
• Participate in wound care if credentialed, following agency policies.

Physical Requirements

Sufficient sight and hearing to comprehend written and verbal communication. Ability to work at least 8 hours daily, involving driving, standing, sitting, assisting patient movement, writing, and speaking.

Qualifications

• Graduate of an accredited Physical Therapy program, Bachelor’s degree or higher.
• Current Texas license to practice as a Physical Therapist.
• Minimum two years of relevant experience; home health experience preferred.
