Boost Home Healthcare in Fort Collins, CO seeks a Home Health Physical Therapist (PT) for full-time, part-time, or PRN roles. You will deliver physician-prescribed PT in patients' homes, perform initial assessments, develop care plans, and conduct 30-day reassessments.

Candidates must have a PT license, be proficient with Medicare OASIS documentation and care planning, and have at least 1 year of home health experience. We offer a flexible schedule and opportunities for advancement.

Qualifications

• Graduate with a PT degree from an accredited program (CAPTE/APTA).
• Active PT license in the practice state.
• Minimum 1 year of home health experience preferred.
• Proficient with Medicare OASIS documentation and care planning.

Responsibilities

• Deliver physician-prescribed physical therapy to help patients regain independence.
• Perform initial assessments, create and adjust care plans, and conduct 30-day reassessments.
• Complete Medicare OASIS documentation in a timely manner.
• Collaborate with interdisciplinary home care team during planning.
• Direct and supervise clinical personnel as required.
