Job summary

Boost Home Healthcare is seeking a licensed physical therapist to provide physician-prescribed PT in a home health setting. You will assess, treat, and progress patients toward independence, coordinating with the care team to optimize outcomes.

The role supports various schedules (PRN, part-time, or full-time) with a per-visit compensation model of $68.00–$76.00 and flexible opportunities for advancement within a growing practice.

Qualifications

Graduate with a Bachelor’s, Master’s, or Doctorate in physical therapy from an accredited program.
Current license in the practicing state.
One year of home health experience preferred.
Knowledge of Medicare OASIS documentation.

Responsibilities

Provide physician-prescribed physical therapy.
Improve or minimize residual physical disabilities.
Return the patient to optimum, productive level within capability.
Collaborate with home care team on patient care planning; supervise staff as needed.
Perform initial assessment, develop and modify plan of care, and reassess every 30 days.

What You’ll Be Doing

Provide physician prescribed physical therapy.
Improve or minimize residual physical disabilities of the patient.
Return the individual to optimum and productive level within the patient’s capability.
Participate with all other home care personnel in patient care planning.Direct and supervise personnel as required.
Responsible for initial assessment, plan of care, maintenance program development and modifications and reassessments every 30 days.

What We’re Looking For

Graduate with a Bachelor’s, Master’s, or Doctorate in physical therapy from a physical therapist curriculum approved by the Commission on Accreditation in Physical Therapy Education, American Physical Therapy Association, or the Committee on Allied Health Education and Accreditation of the American Medical Association, or the Council of Medical Education of the American Medical Association and American Physical Therapy Association.
Currently licensed in the state in which practicing
One (1) year experience preferred in a home health setting
Knowledgeable and experience with Medicare OASIS documentation
