Location: Elma, Washington. Employment type: Full-time. Work shift: All non union, non exempt staff.

Job Summary

Responsible for planning, implementing, and evaluating physical therapy services for patients in concert with the total care plan. Provide quality care to attain patient’s optimal functional level.

Responsibilities and Duties
  • Provide comprehensive Physical Therapy care in outpatient and inpatient settings, including intakes and evaluations, goal setting with patients, implementing care plans, educating patients about risks and benefits, and monitoring progress. Collaborate with the patient care team to formulate the total care plan.
  • Develop care plans based on assessment data using a problem-oriented format; prepare and instruct PT approaches; adjust plans according to patient understanding and progress.
  • Ensure implementation and compliance of the therapy plan; modify treatment plans as indicated; instruct caregivers and family for maximal function and safety at home; make referrals to other disciplines as appropriate.
  • Evaluate the therapy plan and modify as needed at least every 30 days; obtain feedback from team members; discuss progress with patient/family and attend to alterations in the plan with the attending physician as needed.
  • Meet or exceed quality care standards, contribute to quality improvement, and strive for high patient satisfaction. Plan work independently and participate in the quality improvement system while complying with applicable standards and safety goals.
  • Maintain professional skills through ongoing education and training; participate in interdisciplinary meetings; stay current with trends and best practices; ensure compliance with regulatory requirements.
  • Perform special projects and other related duties as assigned.
Essential Functions

Education and Experience
  • Clinical Doctorate, Master’s, or Bachelor’s degree in Physical Therapy.
  • One year of physical therapy practice preferred.
Licenses, Certifications, and Registrations
  • Current and unrestricted license to practice Physical Therapy in the state of Washington.
  • Current BLS certification.
Knowledge, Skills, and Abilities
  • Comprehensive knowledge of therapy theories and concepts.
  • Professional written and verbal communication; ability to tailor communication to audience.
  • Ability to collaborate with providers, other health care professionals, managers, and patients.
  • Accurate and timely documentation and record-keeping.
  • Ability to work independently with strong judgment, problem-solving, critical thinking, organization, and prioritization.
