Flagship Therapy is seeking a Full Time Physical Therapist at Madera Post Acute Center in El Monte, CA. Pay ranges from $54 to $58 per hour based on experience, with a flexible full-time schedule focused on quality post-acute rehab services.

The role involves evaluating patients, delivering skilled therapies, and contributing to discharge planning and family counseling, all within CAPLICO values. New grads are welcome to apply.

Qualifications

• Physical Therapist license is required.

Responsibilities

• Screen and evaluate patients with physical functioning disorders and/or wound care; develop care plans.
• Deliver rehabilitative and medically necessary treatment interventions.
• Administer procedures as part of the rehabilitation plan including manual techniques, ambulation, therapeutic exercises, modalities, devices, and wound care.
• Provide counseling to patients, families, and caregivers.
• Generate discharge summaries ensuring regulatory and clinical compliance.

Madera Post Acute Center in El Monte, CA, is a post-acute care facility with long-term care, short-term care, and geriatric outpatient services provided. We can offer a stable and fun in-house therapy team to work with as well as excellent programs serving our geriatric clientele in their journey to recovery. We are currently seeking a Full Time Physical Therapist for our in-house rehab program!
Full Time: Flexible schedule with work-life balance
Pay: $54 - $58/hour based on experience

Key Responsibilities:

• Conduct effective screening and evaluation of patients with physical functioning disorders and/or wound care, developing appropriate care plans adhering to regulatory and clinical standards.
• Deliver rehabilitative, skilled, and medically necessary treatment interventions to patients with physical functioning disorders and/or wound care, in accordance with regulatory and clinical practice requirements.
• Administer various procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, use of supportive and assistive devices, and wound care.
• Provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders.
• Generate comprehensive discharge summaries of services provided, ensuring compliance with regulatory and clinical requirements.

Qualifications:

Physical Therapist license is required.
