Job summary

Fairview Park Hospital in Dublin, GA is seeking a dedicated Physical Therapist to help patients regain strength, mobility, and confidence after illness, injury, or surgery. You will work with physicians and the rehab team to assess needs and design therapy plans focused on meaningful goals and patient outcomes.

The role offers specialty services such as lymphedema, vestibular, aquatic therapy, BIG program for Parkinson, and a 1:1 mentorship program.

Qualifications

  • Basic Cardiac Life Support must be obtained within 30 days of employment start date.
  • Licensed Physical Therapist.
  • Doctorate Degree, or Bachelors Degree, or Masters Degree.
  • One year of experience as Physical Therapist in a hospital setting is preferred.

Responsibilities

  • Provides evaluations and treatment interventions within the scope of practice established by law and professional code of ethics.
  • Assesses and evaluates therapeutic/rehabilitative/functional status, and participates in the development of the total plan of care.
  • Provides purposeful and skilled therapeutic care at patient’s home, using procedures and techniques to produce a change in condition/function consistent with the physician’s plan of care.
  • Observes and reports patient’s reaction to treatment and changes in patient’s condition.
  • Instructs patient/family/caregiver in the use of therapies, exercises, therapeutic appliances, and/or prosthetic devices to be used outside of therapy sessions.
  • Ensures accurate, complete, and timely clinical documentation in accordance with service/department guidelines.
  • Communicates pertinent payor needs/requirements to physician, treatment team, patient and patient’s family in a timely manner.
  • Prioritizes caseload and, as appropriate, determines daily staffing needs and coordinates assignments/handoffs. Demonstrates efficient and appropriate use of support staff (technicians, PTA’s, other clinicians, and office staff).
  • Provides a safe, secure, and infection/hazard-free environment.
  • Serves as an educational resource to patients, families, interdisciplinary team members, payor representatives, physicians, discharge planners, therapists, community members, and other parties as appropriate regarding rehabilitation issues.
  • Maintains current knowledge of and adherence to federal, state, and local regulations.
  • Assumes accountability for promoting consistent, positive patient interactions that advance the agenda of unparalleled patient service.
  • Maintains a patient-first philosophy, and supports the development and implementation of strategies to elevate the patient experience.
  • Participates as an engaged, active member of the health care team and positively promotes the rest of the team to patients/families/caregivers and other colleagues.
  • Provides general supervison for PT Assistants, as applicable. Communicates clearly, and leads in a way that inspires commitment to the vision/mission/values of the organization.
  • Performs other duties as assigned.
  • Practices and adheres to the “Code of Conduct” and “Mission and Value Statement.”

Job Summary and Qualifications

As a Physical Therapist , you play a vital role in helping patients regain strength, restore mobility, and rebuild confidence following illness, injury, or surgery. Working closely with physicians and the care team, you’ll assess each patient’s needs and create personalized therapy plans designed around meaningful goals, active engagement, and exceptional patient outcomes. Position Summary: Plans, organizes, and administers Physical Therapy services for patients referred who have physical disabilities within the scope of practice of a licensed Physical Therapist, exercising professional judgement, knowledge, and skill in selection and execution of assessments and treatment plans. Participates in an interdisciplinary team to plan and implement Physical Therapy services, staff development programs, and program evaluation/quality management/improvement programs. Acts as a member of the interdisciplinary rehab team by participating in team meetings and performs other duties as assigned by the rehab director to provide a more comprehensive and effective rehabilitation process
