Job summary

Choice Health at Home in Denver is seeking a full-time Home Health Physical Therapist to provide one-on-one care in patients' homes, helping them regain mobility and independence.

You will assess, plan, and implement individualized treatments, document progress, and collaborate with the care team, enjoying autonomy and a varied daily visit schedule.

Applicants should hold a Colorado PT license, CPR, a valid driver’s license, and prefer home health experience; Homecare Homebase is a plus.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Colorado Physical Therapist license.
• CPR certification.
• Valid driver’s license and reliable transportation.
• Current automobile insurance.
• Home health experience preferred.
• Homecare Homebase (HCHB) experience is a plus.
• Strong communication, organization, and time-management skills.

Responsibilities

• Perform comprehensive initial evaluations and identify patient needs, functional limitations, and goals.
• Develop and implement individualized plans of care to improve mobility, strength, balance, and function.
• Provide skilled physical therapy interventions in patients’ homes.
• Evaluate patient progress and adjust treatment plans for best outcomes.
• Educate patients and caregivers on exercises, mobility techniques, fall prevention, safety, and progress.
• Collaborate with physicians, nurses, therapists, caregivers, and other care team members.
• Maintain timely clinical documentation per guidelines.
• Follow infection-control and universal-precaution standards.
• Help patients remain safe, independent, and successful at home.

What You’ll Do

• Provide skilled physical therapy interventions in patients’ homes
• Evaluate patient progress and adjust treatment plans to achieve the best possible outcomes
• Educate patients and caregivers on exercises, mobility techniques, fall prevention, safety, and continued progress
• Collaborate with physicians, nurses, therapists, caregivers, and other members of the interdisciplinary care team
• Maintain timely and accurate clinical documentation in accordance with company and regulatory guidelines
• Follow infection-control and universal-precaution standards
• Help patients remain safe, independent, and successful in the place they call home

What We’re Looking For

• Graduate of an accredited Physical Therapy program
• CurrentColorado Physical Therapist license
• CPR certification
