Job summary

Choice Health at Home seeks a compassionate Physical Therapist to join our home health team. You’ll provide personalized therapy to help patients improve mobility, relieve pain, build strength, and regain independence in their own homes.

Responsibilities include evaluations, developing treatment plans, documenting progress, and collaborating with families and healthcare professionals to ensure high-quality patient care. This is a full-time role in Denver with competitive pay and benefits.

Qualifications

• Graduate of an accredited physical therapy program.
• Current, valid Physical Therapist license.
• CPR certification.
• Valid driver’s license and current auto insurance.
• At least one year of home health experience preferred.
• Homecare Homebase experience preferred.

Responsibilities

• Perform and document comprehensive initial evaluations.
• Identify functional problems and establish appropriate diagnoses before intervention.
• Develop and implement individualized treatment plans.
• Help patients maintain, improve, or restore physical function.
• Provide treatments designed to alleviate pain and prevent further dysfunction.
• Evaluate patient progress and adjust treatment plans to achieve the greatest possible benefit.
• Follow infection-control standards and universal precautions.
• Maintain accurate, timely documentation in accordance with company guidelines.
• Ensure continuity and quality of patient care.
• Communicate effectively with patients, families, team members, physicians, and other healthcare professionals.

Choice Health at Home is seeking a compassionate and motivated Physical Therapist to join our home health team. In this role, you’ll provide personalized therapy that helps patients improve mobility, relieve pain, build strength, and regain independence in the comfort of their own homes.

Physical Requirements

This position requires the ability to:
