Heights Care & Rehabilitation is seeking a compassionate Physical Therapist (PT) to help residents regain mobility and independence. You’ll assess needs, develop plans, and provide evidence-based care in a team-focused environment.

The role supports flexible scheduling with full-time, part-time, and PRN options, including a $5,000 sign-on bonus paid at milestones, and a strong culture of resident-centered care.

Qualifications

• Current Physical Therapist license in the State of Colorado.
• Graduation from an accredited Physical Therapy program.
• Strong clinical reasoning and assessment skills.
• Excellent communication and teamwork abilities.
• New graduates encouraged to apply.

Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Provide evidence-based physical therapy interventions.
• Improve strength, balance, mobility, endurance, and functional independence.
• Collaborate with physicians, nursing staff, occupational therapists, speech therapists, and families.
• Assist with discharge planning and safe transitions home.
• Educate residents, families, and caregivers on mobility and safety.
• Maintain accurate and timely documentation.
• Participate in interdisciplinary care planning meetings.
• Help create a positive, motivating therapy environment where residents look forward to treatment.

What You'll Do

What We're Looking For
