Job summary

Full Circle Therapy and Wellness in De Pere, WI seeks a compassionate physical therapist to join our clinic full-time, part-time, or PRN. We focus on patient-centered care in a collaborative environment where clinicians and patients thrive.

You will evaluate movement, design personalized treatments, and provide 45-minute one-on-one sessions, educating patients on self-care and wellness, working with PTAs, and ensuring accurate EMR documentation. A license in Wisconsin is required.

Qualifications

• DPT degree from an accredited program.
• Current Wisconsin Physical Therapist license.
• Excellent communication and interpersonal skills.
• Proven track record for patient progress.
• Strong clinical reasoning and problem-solving abilities.
• Dry needling certification and experience highly desirable.

Responsibilities

• Perform comprehensive patient evaluations to assess movement abilities and limitations.
• Develop personalized treatment plans to meet individual goals.
• Provide evidence-based physical therapy interventions in one-to-one, 45-min sessions.
• Educate patients on treatment, exercises, self-care strategies, and long-term wellness.
• Collaborate with PTAs to support treatment and patient care.
• Monitor patient progress and modify treatment as needed.
• Complete all patient documents within PROMT EMR and maintain compliance.
• Coordinate with physicians and other health professionals for coordinated care.

Full Circle Therapy and Wellness, a small, privately owned outpatient physical therapy clinic in De Pere, WI is seeking a compassionate, motivated and skilled physical therapist to join our team, full-time, part-time or PRN. Responsibilities

• Work with our PTA's to collaborate on treatment and patient care.

• Accurately and timely complete all patient evaluation, treatment and progress notes within PROMT EMR and maintain compliance.
• Collaborate with physicians and other healthcare professionals to provide exceptional coordinated patient care.
