Job summary

Homeside Rehab is seeking a compassionate Pediatric Physical Therapist or PTA to join our home health team. You will deliver individualized, evidence-based therapy to pediatric patients in their homes, offering schedule flexibility and a supportive team environment.

The role requires a CT PT license, strong clinical assessment, excellent communication with families, and the ability to work independently during home visits. Prior pediatric/home health experience is preferred.

Qualifications

• Active CT Physical Therapy License (PTA) — required.
• Basic computer proficiency for electronic documentation.
• Strong clinical assessment and interpersonal communication skills.
• Ability to work independently in a home health environment.
• CT driver's license and reliable transportation for home visits.
• Pediatric experience is preferred.
• Minimum 1 year of experience in Home Health, Outpatient, or SNF settings.

Responsibilities

• Evaluate patients and develop individualized treatment plans based on clinical assessment and patient goals
• Provide evidence-based PT interventions to improve motor skills, functional independence, and developmental outcomes
• Maintain accurate, timely documentation of all patient care in accordance with regulatory requirements
• Collaborate with the clinical team to ensure coordinated and effective treatment
• Communicate clearly with office staff, patients, and caregivers regarding progress and care plans

Job Responsibilities
