Job summary

Homeside Rehab in Pennsylvania is seeking a compassionate Pediatric Physical Therapist to join our pediatric home health team. You will deliver individualized, evidence-based therapy to pediatric patients in their homes, helping families reach developmental goals with flexible scheduling.

The role emphasizes collaboration with the clinical team, thorough documentation, and independent home visits. We offer Monday-Friday hours with some weekend shifts as needed, and competitive hourly pay between

Qualifications

• PA Physical Therapy license required.
• Basic computer proficiency for electronic documentation.
• Strong clinical assessment and interpersonal communication skills.
• Ability to work independently in a home health environment.
• PA driver's license and reliable transportation for home visits.
• Pediatric experience preferred.
• Minimum 1 year of Home Health/Outpatient/SNF experience.

Responsibilities

• Evaluate patients and develop individualized treatment plans based on clinical assessment and patient goals
• Provide evidence-based PT interventions to improve motor skills, functional independence, and developmental outcomes
• Maintain accurate, timely documentation of all patient care in accordance with regulatory requirements
• Collaborate with the clinical team to ensure coordinated and effective treatment
• Communicate clearly with office staff, patients, and caregivers regarding progress and care plans

Job Responsibilities

• Evaluate patients and develop individualized treatment plans based on clinical assessment and patient goals
• Provide evidence-based PT interventions to improve motor skills, functional independence, and developmental outcomes
• Maintain accurate, timely documentation of all patient care in accordance with regulatory requirements
• Collaborate with the clinical team to ensure coordinated and effective treatment

Job Qualifications

• Active Pennsylvania Physical Therapy License (PTA) — required
• Basic computer proficiency for electronic documentation
• Strong clinical assessment and interpersonal communication skills
• Ability to work independently in a home health environment
• Valid Pennsylvania driver's license and reliable transportation for home visits
• Pediatric experience is preferred
• Minimum 1 year of experience in Home Health, Outpatient, or Skilled Nursing Facility (SNF) settings

Job Details

• Salary: $65 - $80 per hour
• Schedule Flexibility: Choose your own hours, day and evening shifts available, Monday to Friday, with weekend availability as needed
