Responsibilities

  • Evaluate patients to assess strength, balance, mobility, and functional impairments.
  • Develop and implement personalized treatment plans that focus on improving mobility and reducing pain.
  • Deliver care at one or more assisted living locations, providing meaningful services to a diverse group of seniors.
  • Provide therapeutic interventions such as manual therapy, therapeutic exercises, gait training, and balance programs.
  • Promote safety and fall prevention by identifying risks and implementing preventive measures.
  • Provide care with a high level of independence in assisted living communities, while supported by the Engage team for guidance and resources.
  • Educate patients and families on home exercise programs and strategies for maintaining functional independence.
  • Document patient care services in EMR in compliance with state and facility standards.
  • Enhance professional growth through participation in educational programs, in-service meetings, and workshops.
  • Develop bonds with patients, families, doctors, and team members to ensure collaborative care and successful outcomes.

As a Physical Therapist, you will:
