You will use EMR for documentation and may access AI scribing tools to streamline notes while pursuing ongoing professional growth.

Qualifications

• Graduation from an accredited physical therapy program.
• Active or pending state PT license.
• Commitment to evidence-based patient care.

Responsibilities

• Evaluate patients to assess strength, balance, mobility, and impairments.
• Develop and implement individualized treatment plans to improve function.
• Deliver care at assisted living locations, with independence and team support.
• Provide interventions such as manual therapy, therapeutic exercise, gait training, and balance programs.
• Educate patients and families on home exercises and independence strategies.
• Document care in EMR in compliance with state and facility standards.

As a Physical Therapist, you will:

• Evaluate patients to assess strength, balance, mobility, and functional impairments.
• Develop and implement personalized treatment plans that focus on improving mobility, reducing pain, and enhancing overall function.
• Deliver care at one or more assisted living locations, providing meaningful services to a diverse group of seniors.
• Provide therapeutic interventions such as manual therapy, therapeutic exercises, gait training, and balance programs.
• Promote safety and fall prevention by identifying risks and implementing preventive measures.
• Provide care with a high level of independence in assisted living communities, while always supported by the broader Engage team for guidance and resources.
• Educate patients and families on home exercise programs and strategies for maintaining functional independence.
• Document patient care services in compliance with state and facility standards using EMR.
• Enhance professional growth and development through participation in educational programs, in-service meetings, and workshops.
• One-on-one personalized patient care.
• Develop bonds not just with patients, but with their families, doctors, and fellow team members, ensuring collaborative care and successful outcomes.

• Graduation from an accredited physical therapy program.
• Active or pending state license to practice PT.
