You will collaborate with multidisciplinary teams, document care in EMR, and educate patients and families on mobility and prevention strategies. Licensure as a Physical Therapist and experience in acute care or home health are preferred.

*Duties

• Conduct comprehensive patient assessments to determine functional limitations and develop tailored treatment plans.
• Implement evidence-based physiotherapy techniques, including trigger point therapy and orthopedics interventions, to facilitate recovery.
• Collaborate with multidisciplinary teams to coordinate discharge planning and ensure seamless transitions between inpatient, outpatient, and home health care.
• Monitor patient progress regularly, adjusting care plans as necessary to optimize outcomes.
• Educate patients and their families on exercises, injury prevention, and health maintenance strategies within home and community care settings.
• Document all patient interactions accurately using electronic medical record (EMR) systems while maintaining confidentiality.
• Provide specialized care for diverse populations including pediatrics, geriatrics, and individuals recovering from orthopedic or neurological conditions.

*Requirements

• Valid licensure or certification as a licensed Physical Therapist in the relevant jurisdiction.
• Proven experience in acute care, hospital environments, outpatient clinics, or home health services.
• Strong knowledge of anatomy, physiology, and medical terminology relevant to physiotherapy practice.
• Experience developing comprehensive care plans that incorporate discharge planning and patient-centered goals.
• Familiarity with EMR systems for documentation and patient record management.
