CenterCare Home Health is hiring Physical Therapists to join a supportive team serving Colorado Springs and surrounding areas. Part-time or PRN positions offer flexible scheduling and competitive pay, with full benefits and opportunities for mentorship.

As a PT, you will evaluate and treat patients in their homes, help improve mobility, strength, and safety, and document care in Kantime EMR. Requires CO license and 1+ year in acute or home health; Medicare home health experience is preferred.

Qualifications

• Must hold a Colorado PT license.
• 1+ year of experience in acute care or home health.
• Medicare home health experience is preferred.
• Passion for patient-centered care.
• EMR experience is required.

Responsibilities

• Evaluate and treat patients in the home.
• Help improve mobility, strength, and safety for patients.
• Document care using EMR (Kantime a plus).
