Responsibilities

• Evaluate residents and develop individualized physical therapy treatment plans
• Provide therapeutic interventions including strength training, balance training, gait training, and mobility exercises
• Assist residents in improving functional mobility, transfers, and ambulation
• Collaborate with interdisciplinary care teams including OT, SLP, nursing staff, and physicians
• Educate residents and caregivers on safety, fall prevention, and home exercise programs
• Participate in care planning meetings and discharge planning

Job Overview

We are seeking a compassionate and dedicated Physical Therapist (PT) to join our rehabilitation team within a Skilled Nursing Facility (SNF) setting. The Physical Therapist will work closely with residents recovering from illness, injury, or surgery to restore mobility, improve strength and balance, and help patients achieve the highest possible level of independence. This role is ideal for therapists who enjoy working with the geriatric population and collaborating with an interdisciplinary care team.
