Job summary

Mosaic Health, LLC. seeks a licensed Physical Therapist to join our Home Health Team in Cape Coral, Florida. This part-time, PRN role offers flexible hours and focuses on delivering personalized in-home rehabilitation.

You will assess mobility, design treatment plans, provide therapeutic interventions, educate patients and families, and document care while ensuring HIPAA compliance. A Florida PT license and at least 1 year of clinical experience are preferred.

Qualifications

Responsibilities

In this role, there is a real impact in the comfort of patients’ homes. As part of our compassionate, high‐performing care team, you will evaluate patients, develop individualized treatment plans, and provide therapeutic interventions to improve mobility, strength, balance, and functional independence. You will collaborate with an interdisciplinary care team, educate patients and families on rehabilitation and injury prevention strategies, maintain accurate clinical documentation, and ensure compliance with all regulatory standards to deliver high-quality patient care and achieve optimal outcomes.
