Skyline Ridge Health and Rehabilitation in Colorado is seeking a dedicated Physical Therapist to assess patients, develop care plans, and provide rehabilitative treatment for physical functioning disorders and wounds.

The role offers $43-$52 per hour, full-time, on-site. You will screen, implement interventions, document progress, and coordinate with supervising PT and care teams. New grads are welcome; licensed PT required.

Duties

• Conduct effective screening and evaluation of patients with physical functioning disorders and/or wound care, developing appropriate care plans adhering to regulatory and clinical standards.
• Deliver rehabilitative, skilled, and medically necessary treatment interventions to patients with physical functioning disorders and/or wound care, in accordance with regulatory and clinical practice requirements.
• Administer various procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, use of supportive and assistive devices, and wound care.
• Provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders.
• Generate comprehensive discharge summaries of services provided, ensuring compliance with regulatory and clinical requirements.

Duties

• Utilizes evidence-based practice to support clinical interventions.
• Provides rehabilitative, skilled and medically necessary treatment intervention to patients with physical functioning disorders per regulatory and clinical practice requirements under the supervision and direction of a Physical Therapist.
• Administers various procedures as part of the rehabilitation plan including but not limited to: manual techniques; ambulation; therapeutic exercises; modalities; use of supportive and assistive devices; etc.
• Under the supervision and direction of a Physical Therapist
• Assesses patient’s progress and communicates with supervising Physical Therapist to adjust treatments accordingly in the clinical record per regulatory and clinical practice requirements
• Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to the physical disorders
• Provides input to supervisor for discharge summary of services provided per regulatory and clinical requirements
• Documents all regulatory and clinically required information in the electronic documentation system and maintains patient records according to company policy.
