Skyline Ridge Health and Rehabilitation in Canon City, CO is seeking a licensed Physical Therapist (PT) to join our full-time team. The role offers hourly pay between $43 and $52 and opportunities for professional growth.

You will evaluate patients with physical function disorders, develop care plans, provide rehabilitative interventions, and document progress in the medical record. We welcome new grads and support a diverse, inclusive workplace.

Qualifications

• Physical Therapy license is required.
• Open to PTs at all experience levels; New grads welcome.

Responsibilities

• Screen and evaluate patients with physical functioning disorders and wound care; develop care plans.
• Deliver rehabilitative, skilled, and medically necessary treatment interventions.
• Administer procedures including manual techniques, ambulation, exercises, modalities, assistive devices and wound care.
• Provide consultation and counseling to patients, families, and other providers.
• Generate discharge summaries complying with regulatory and clinical requirements.
• Document all clinical information in the electronic system per policy.

Job Title: Physical Therapist (PT)

Schedule: Full-time

Duties:

• Conduct effective screening and evaluation of patients with physical functioning disorders and/or wound care, developing appropriate care plans adhering to regulatory and clinical standards.
• Deliver rehabilitative, skilled, and medically necessary treatment interventions to patients with physical functioning disorders and/or wound care, in accordance with regulatory and clinical practice requirements.
• Administer various procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, use of supportive and assistive devices, and wound care.
• Provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders.
• Generate comprehensive discharge summaries of services provided, ensuring compliance with regulatory and clinical requirements.

Qualifications:

• Physical Therapy license is required.

Duties:

• Utilizes evidence-based practice to support clinical interventions.

• Provides rehabilitative, skilled and medically necessary treatment intervention to patients with physical functioning disorders per regulatory and clinical practice requirements under the supervision and direction of a Physical Therapist.

• Administers various procedures as part of the rehabilitation plan including but not limited to: manual techniques; ambulation; therapeutic exercises; modalities; use of supportive and assistive devices; etc. Under the supervision and direction of a Physical Therapist

• Assesses patient’s progress and communicates with supervising Physical Therapist to adjust treatments accordingly in the clinical record per regulatory and clinical practice requirements

• Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to the physical disorders

• Provides input to supervisor for discharge summary of services provided per regulatory and clinical requirements

• Documents all regulatory and clinically required information in the electronic documentation system and maintains patient records according to company policy
