Job summary

CareCore at Willowood in Brunswick, OH is seeking a compassionate PRN Physical Therapist to join our in-house rehabilitation team serving residents in a skilled nursing and long-term care setting.

This PRN role offers flexible weekday and weekend opportunities. You will collaborate with physicians, nursing staff, and families, document with Net Health, and use PCC within the interdisciplinary care process to support resident mobility and independence.

Qualifications

• Current Ohio PT license in good standing.
• Graduate of an accredited physical therapy program.
• Experience in skilled nursing or long-term care preferred.

Responsibilities

• Perform comprehensive physical therapy evaluations.
• Develop and implement individualized treatment plans.
• Provide evidence-based therapy to help residents improve mobility, strength, balance, and functional independence.
• Collaborate with physicians, nursing staff, therapy team members, residents, and families.
• Complete accurate and timely documentation using Net Health.
• Utilize PointClickCare (PCC) as part of the interdisciplinary care process.
• Maintain compliance with all federal, state, and company regulations.
