The candidate for the Physical Therapist position will partner with new and ongoing residents to assess the need for, develop and deliver services; and be able to identify the need for and make referral(s) to other disciplines. In addition, he/she will provide instruction, education and training for residents, family members and caregivers in those skills and functions essential to well-being. He/she will facilitate activities to promote and maintain health and quality of life.
Essential Functions
An individual must be able to successfully perform the essential functions of this position with or without reasonable accommodation.
- As necessary or applicable, provide supervision to Physical Therapy Assistants, temporary licensees, students and support team members in accordance with state licensure requirements and professional standards.
- Responsible for adherence to department and resident treatment schedule, timely clinical and point of care billing documentation requirements, and work-related meetings.
- Attend and contribute to resident care, resident/family conferences and other related meetings.
- Make referrals to other disciplines and agencies to address the comprehensive needs of the resident.
- Screens, assess and evaluate residents, including history, systems review, and application of appropriate tests and measures, synthesis information and interpret examination findings to establish a provide diagnosis education, identify impairments, determine the predicted level of improvement and the time required to achieve it, identify precautions/contraindications and design and plan of care. Develop appropriate treatment goals and methods in collaboration with the resident and caregivers, implement the physical therapy treatment plan, and complete all related documentation and record keeping regarding these services.
- Adapt program and/or plan of care according to the needs of individual resident.
- Consult with other members of the team to select the most appropriate therapeutic regimen consistent with the needs and capabilities of each resident.
- Promote a safe environment; Demonstrate knowledge of responsible use of facility equipment, and best practice for equipment use.
- Exhibit resident centered care: ensuring that residents and families receive the highest quality of service in a caring and compassionate atmosphere which recognizes the individuals' needs and rights.
- Assessment for, fitting of and/or application of adaptive devices; and/or trains resident/caregivers in using adaptive devices included orthotics and prosthetics.
- Select appropriate activities suited to the individual's physical, cognitive and emotional capacity, and interest.
- Perform routine reexamination as needed/required to modify/progress plan of treatment.
- Incorporate health and wellness programming into treatment plans for residents
Requirements
- Must have a valid Physical Therapy license in the state(s) of practice, or proof of license eligibility
An individual must be proficient in each of the competencies listed below to successfully perform the responsibilities of this position:
- Effective communication, organizational and interpersonal skills are crucial. Therapist must work closely with residents, families, and other team professionals.
- Ability to work independently, handle specific deadlines and several tasks simultaneously.
- Ability to learn and independently manage multiple medical electronic documentation systems and basic systems such as Outlook, Microsoft Word, Excel and 365 Office applications.
- Must have a valid driver's license.
