Job summary

Visiting Nurse and Hospice for VT & NH is seeking a Physical Therapist Assistant to contribute to patient care in a home health setting. You will work with the Physical Therapist to screen, evaluate, re-evaluate, and treat patients referred by a licensed physician, while coordinating with the care team and patient to ensure effective treatment.

Applicants must hold an active PTA license or be license eligible in VT and NH, and have current CPR certification.

Qualifications
