Skyline Transitional Care in Boise, Idaho is seeking a Physical Therapist for PRN coverage to join our in-house rehab program and support geriatric patients on their recovery journey. The role requires a Physical Therapist license and welcomes new grads.

You will screen, evaluate and treat patients, develop care plans, and provide discharge summaries, with emphasis on evidence-based practice and regulatory compliance.

Qualifications

• Physical Therapist license is required.
• Open to PTs at all experience levels; new grads welcome.

Responsibilities

• Screen and evaluate patients with physical functioning disorders and wound care; develop care plans.
• Deliver rehabilitative treatments to patients in accordance with clinical standards.
• Assist with standing, walking, lifting, transferring, and repositioning of patients and equipment.
• Administer procedures as part of the plan, including manual therapy, exercises, modalities, devices, and wound care.
• Provide counseling to patients, families, and caregivers regarding physical disorders.
• Generate discharge summaries compliant with regulatory requirements.

Skyline Transitional Care in Boise, ID is a post-acute care facility with long-term care, short-term care, and geriatric services provided. We can offer a stable and fun in-house therapy team to work with as well as excellent programs serving our geriatric clientele in their journey to recovery. We are currently seeking a dedicated Physical Therapist – PRN for our in-house rehab program!

PHYSICAL THERAPIST – PRN

Key Responsibilities

• Conduct effective screening and evaluation of patients with physical functioning disorders and/or wound care, developing appropriate care plans adhering to regulatory and clinical standards.
• Deliver rehabilitative, skilled, and medically necessary treatment interventions to patients with physical functioning disorders and/or wound care, in accordance with regulatory and clinical practice requirements.
• The role involves regular standing and walking throughout the facility and occasional lifting, transferring, and repositioning of patients and equipment. Physical demands vary throughout the day based on patient care needs.
• Administer various procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, use of supportive and assistive devices, and wound care.
• Provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders.
• Generate comprehensive discharge summaries of services provided, ensuring compliance with regulatory and clinical requirements.

Qualifications
