Job summary

Skyline Transitional Care in Boise, ID is a post-acute care facility offering long-term and geriatric services. We are seeking an Occupational Therapist (PRN) to join our in-house therapy team and support recovery journeys.

You will screen and evaluate patients, develop care plans, provide therapeutic interventions, assist with discharge summaries, and collaborate with families and other providers. OT license required; new grads welcome; benefits include health, 401k, PTO, CEUs.

• Generate comprehensive discharge summaries per regulatory and clinical requirements.

OCCUPATIONAL THERAPIST – PRN

Skyline Transitional Care in Boise, ID, is a post-acute care facility with long-term care, short-term care, and geriatric services provided. We can offer a stable and fun in-house therapy team to work with as well as excellent programs serving our geriatric clientele in their journey to recovery.

Key Responsibilities

• Conduct effective screening and evaluation of patients with physical functioning disorders and/or wound care, developing appropriate care plans adhering to regulatory and clinical standards.
• Deliver rehabilitative, skilled, and medically necessary treatment interventions to patients with physical functioning disorders and/or wound care, in accordance with regulatory and clinical practice requirements.
• The role involves regular standing and walking throughout the facility and occasional lifting, transferring, and repositioning of patients and equipment. Physical demands vary throughout the day based on patient care needs.
• Administer various procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, use of supportive and assistive devices, and wound care.
• Provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders.
• Generate comprehensive discharge summaries of services provided, ensuring compliance with regulatory and clinical requirements.

Qualifications

• Occupational Therapist license is required.
• Open to OTs at all experience levels. New Grads are welcome!
