Job summary

Good Shepherd Lutheran Community in Blair, Nebraska is seeking a compassionate Physical Therapist (PT) to join our rehabilitation team in a Full-Time or Per Diem capacity. You will evaluate residents and develop individualized treatment plans to improve mobility, strength, balance, and independence while collaborating with an interdisciplinary team.

Qualifications

• DPT or Bachelor's/Master's in Physical Therapy from an accredited program.
• Current Nebraska Physical Therapist license in good standing.
• Strong clinical reasoning, communication, organizational, and time‑management skills.
• Experience in skilled nursing, senior living, rehabilitation, geriatrics, or similar setting preferred but not required.
• New graduates with appropriate licensure are welcome to apply.

Responsibilities

• Evaluate residents' physical function, mobility, strength, balance, endurance, and functional limitations.
• Develop individualized PT treatment plans based on residents' needs and goals.
• Provide skilled interventions to improve strength, balance, coordination, endurance, mobility and independence.
• Perform gait training, transfer training, therapeutic exercises, neuromuscular re‑education, and functional mobility training.
• Assess and recommend assistive devices and adaptive equipment.
• Educate residents, families, and caregivers on exercises, mobility, fall prevention, safety, and home programs.
• Monitor progress and modify plans as needed.
• Collaborate with physicians, nursing staff, and other rehab professionals.
• Participate in care planning, discharge planning, and clinical activities.
• Maintain accurate documentation per policies and regulations.
• Promote safety, dignity, independence, and quality of life.
