Job summary

The Center for Living and Rehab is seeking a PRN Physical Therapist to join our in-house rehab team. You will evaluate residents, develop individualized treatment plans, and deliver skilled therapy to improve mobility and function within a resident-centered, team-oriented setting.

We value strong communication and clinical skills, and you will document progress in our EMR system while collaborating with an interdisciplinary care team to ensure optimal resident outcomes.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Active Vermont Physical Therapist license (required).
• Experience in SNF or long-term care preferred.
• Strong communication, organizational, and clinical skills.
• Experience with EMR systems preferred.

• Document evaluations, treatment sessions, and outcomes accurately and timely using electronic medical records
• Collaborate with interdisciplinary team members to support resident care plans
