Job summary

Lancaster-Nursing-and-Rehabilitation-Center is seeking a licensed Physical Therapist (PT) to join our in-house rehabilitation team. This full-time opportunity offers a collaborative, patient-centered care approach in a supportive environment.

You will evaluate and treat residents and patients, develop individualized plans, and coordinate with an interdisciplinary team to promote mobility, pain relief, and independence. Commitment to documentation and compliant care is required.

Qualifications

• Current Physical Therapist license (state-specific).
• Graduate of an accredited Physical Therapy program (DPT preferred).
• Strong clinical assessment and documentation skills.
• Excellent communication and teamwork abilities.
• Experience in skilled nursing or long-term care preferred.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Plan and administer therapeutic interventions to relieve pain and improve mobility.
• Restore or enhance functional movement and independence.
• Ensure all treatments follow the established plan of care.
• Monitor and document patient progress and response to treatment.
• Educate patients, families, and caregivers on therapy plans and goals.
• Maintain accurate documentation in compliance with state and federal regulations.

Position Overview

As a Physical Therapist, you will evaluate, develop, and implement individualized treatment plans for residents and patients with temporary or long-term physical conditions. You will play a key role in restoring function, promoting healing, and improving overall quality of life.
