Job summary

The VNA Health Group is seeking a compassionate per diem Physical Therapist to join our homecare team in Hudson County, New Jersey. You will evaluate patients in their homes and provide skilled PT services to maximize mobility and independence while collaborating with physicians and the care team.

Responsibilities include developing individualized treatment plans, documenting progress, and educating patients and families.

Qualifications

• NJ Physical Therapy license is required.
• 2-3 years of clinical experience preferred; homecare or community-based experience is a plus.
• Strong interpersonal and communication skills; proficient with digital documentation.

In this role, you will be responsible for evaluating and providing skilled physical therapy services to patients in their homes, empowering them to live safely and independently while maximizing their mobility and quality of life.

Working Conditions/Physical Demand: Combination of indoor and outdoor work, driving, lifting, walking, standing, fine motor hand movement, and frequent use of phone and tablet for documentation.

In compliance with New Jersey’s Pay Transparency Act, the compensation range for this position is USD $84,000 to USD $112,000. When determining an offer, VNA Health Group considers factors such as the scope and responsibilities of the role, the candidate’s work experience, education/training, relevant skills, internal pay equity, and market and organizational conditions.
