Job summary

Community Physical Therapy in the Chicago area is seeking Admit-to-Discharge Therapist to join our PRN team. The role covers outpatient and home care, following patients from evaluation through discharge, with a focus on building a caseload that fits your availability.

We offer a $55/hour rate and flexible Monday–Friday scheduling (7 AM–6 PM). Candidates must hold an active Illinois therapy license and be comfortable working independently while learning TheraOffice and Homecare Homebase.
