Job summary

Community Physical Therapy is seeking an Admit-to-Discharge Therapist on a PRN basis in Illinois. The role involves treating outpatient and home care patients, following patients from evaluation to discharge, with a flexible weekday schedule from 7 AM to 6 PM.

Active Illinois license is required, outpatient or home care experience is preferred, and the candidate should be dependable, flexible, and comfortable working independently. TheraOffice and Homecare Homebase experience is a plus.

Community Physical Therapy (CPT) is looking for a PRN therapist to join our Admit-to-Discharge team!
