Job summary

Presbyterian Homes & Services (PHS) is seeking a Physical Therapist for Mill Pond in Ankeny, IA. The role offers a Monday–Friday schedule with no weekends, enabling you to serve residents across Independent Living, Assisted Living, Memory Care, Long Term Care, and Home Health settings.

You will perform assessments, develop and progress treatment plans, educate residents and families, and coordinate discharges along the therapy continuum, with documentation in an electronic medical record and

Qualifications

• Graduate from an accredited Physical Therapy program.
• Current licensure in the state(s) of practice as required.
• Demonstrated compatibility with PHS's mission and operating philosophies.
• Ability to read, write, speak and understand English to communicate with customers.

Responsibilities

• Conduct a comprehensive assessment and evaluation using subjective and objective measures.
• Develop treatment plans and programs in accordance with physician orders.
• Provide quality treatments, actively progressing goals to achieve highest quality of life.
• Conduct screens and provide education to residents/clients, families, caregivers, and staff.
• Coordinate seamless discharges utilizing the full continuum of therapy and fitness services.
• Provide accurate and timely documentation and billing in the electronic medical record.
• Demonstrate understanding and knowledge of payer sources and regulatory requirements.
• Complete and document supervisory visits per regulations.
• Communicate effectively with residents/clients, families, caregivers, physicians, referral sources, and interdisciplinary team.

Mill Pond is seeking a Full or Part Time Physical Therapist.

SCHEDULE: Monday-Friday, flexible schedule. No weekends!

Responsibilities

As a Physical Therapist you will be responsible to:
