Job summary

Presbyterian Homes & Services (PHS) HallMar Village is seeking an On-Call/PRN Occupational Therapist to join our team in Cedar Rapids, IA. You will conduct comprehensive assessments, develop treatment plans, and deliver quality therapy across a continuum of settings while documenting diligently in the EMR.

Qualifications

• Graduate from an accredited Occupational Therapy program.
• Current licensure in the state(s) of practice as required.
• Demonstrated compatibility with PHS's mission and operating philosophies.
• Demonstrated ability to read, write, speak and understand English.

Responsibilities

• Conduct a comprehensive assessment & evaluation using subjective and objective measures.
• Develop treatment plans and programs in accordance with physician orders.
• Provide quality treatments, actively progressing goals to achieve highest quality of life.
• Conduct screens and provide education to residents/clients, families, caregivers, and staff.
• Coordinate seamless discharges utilizing the full continuum of therapy and fitness services.
• Provide accurate and timely documentation & billing in electronic medical record.
• Demonstrate understanding and knowledge of payer sources and regulatory requirements.
• Complete and document supervisory visits per regulations.
• Communicate effectively with residents/clients, families, caregivers, physicians, referral sources, and interdisciplinary team.

Responsibilities

As an Occupational Therapist you will be responsible to:
