Overview

Presbyterian Homes & Services (PHS) is a nonprofit, faith‑based organization that provides high‑quality housing, care, and services options for older adults. The Home & Community based division of PHS, Optage, is hiring a Full‑Time Physical Therapist Assistant to serve in home‑health care for communities in the south Twin Cities metro area.

Schedule / Hours

Monday through Friday, full‑time (up to 30 hours per week) with a $1,000 hiring bonus.

Responsibilities

• Provide treatment per the established plan of care and in accordance with physician orders.
• Assist in identifying goals, prioritizing treatment, and selecting appropriate interventions.
• Continuously assess changes in functional deficits and adjust treatment as appropriate.
• Provide education to residents, clients, families, caregivers, and staff.
• Collaborate with therapists on observations, progress toward goals, and recommended updates to plan of care and discharge.
• Provide accurate and timely documentation & billing in electronic medical records.
• Demonstrate understanding and knowledge of payer sources and regulatory requirements.
• Coordinate with therapists on supervisory visits per regulations.
• Communicate effectively with residents, clients, families, caregivers, physicians, referral sources, and the interdisciplinary team.

Qualifications

• Graduate from an accredited Physical Therapist Assistant program.
• Current licensure in the state(s) of practice as required.
• Demonstrated compatibility with PHS’s mission and operating philosophies.
• Demonstrated ability to read, write, speak, and understand the English language to communicate with all customers.

Pay Range

Estimated starting pay range: $30.24 - $34.57 per hour. Actual pay depends on qualifications and market conditions.
