Job summary

PurposeCare is seeking a licensed Physical Therapist to join our home health team, delivering high-quality care in patients' homes with flexible scheduling and autonomy.

Join a team focused on personalized care and real-life therapy, helping patients achieve meaningful progress in daily life and recovery through compassionate, in-home visits.

Qualifications

• Current Physical Therapist license in the state you are applying (Indiana).
• 1 year of physical therapist experience preferred - home health and OASIS experience preferred.
• Ability to perform tasks involving physical activity, up to 50 lbs, bending and standing.
• Current Driver's License, vehicle insurance, and/or reliable transportation.
• Ability to successfully complete a background check.

Responsibilities

• Provide skilled PT visits in patients' homes.
• Develop and implement home-based treatment plans.
