PurposeCare is seeking a licensed Physical Therapist to provide home health services across the Midwest. You will deliver skilled visits in patients' homes, develop and implement individualized plans, and collaborate with care teams to support recovery and independence.

This role offers flexible scheduling, autonomy in clinical decisions, and ongoing training. A valid PT license, drivers license, and the ability to lift up to 50 lbs are required to help patients stay healthy and thrive in their

• Current Physical Therapist license in the state of application.
• 1 year of physical therapist experience preferred - home health and OASIS experience preferred.
• Ability to perform tasks involving physical activity, up to 50 lbs, bending, and standing.
• Valid drivers license, vehicle insurance, and/or access to dependable transportation.
• Ability to successfully complete a background check.
