Purpose of this position

The position involves evaluating, treating, documenting, and facilitating care for residents with impairments affecting their independence due to disease. You will participate in interdisciplinary team meetings and assist in developing decline prevention policies.

Essential Job Duties & Responsibilities

• Evaluate and document resident assessments according to facility policies.
• Provide treatments to help residents maintain or improve their function.
• Recommend and provide adaptive equipment and preventative measures.
• Maintain accurate reports for regulatory agencies and payers.
• Observe infection control procedures and ensure resident safety.
• Complete assessments, care plans, and documentation timely.
• Participate in care conferences and quality improvement programs.
• Collaborate effectively with residents, families, staff, and healthcare professionals.
• Follow all policies, safety procedures, and residents' rights.
• Adhere to compliance and confidentiality standards.
• Present a professional appearance, demonstrate dependability, and perform duties effectively.
• Support a cooperative work environment and accept additional duties as assigned.

Qualifications

• Excellent verbal and written communication skills.

• Proficiency with Word and Excel.
