Iconic Care is seeking a full-time physiatrist to provide autonomous care at Skilled Nursing Facilities in California. You will round at multiple facilities and manage a 23–25 patient daily load, focusing on improving participation in therapy and functional recovery.

The role offers a flexible 5‑day workweek, no nights, and competitive production compensation with robust benefits. Relocation assistance is provided as part of the package.

Qualifications

• Current, unrestricted PA or NP license.
• DEA license or willingness to obtain DEA licensure.
• Experience in nursing home/physiatry is a plus but not required.
• Comfort with peripheral joint injections (knees, shoulders, hips).
• Strong communication and collaboration with multidisciplinary teams.
• Familiarity with EHR charting.

Responsibilities

• Rounding at 1–3 SNFs or rehab centers 1–3 days per week per location.
• Treat 25+ patients per day with participation challenges due to pain or functional limits.

Day-to-Day Responsibilities

As a physiatrist, you will round at 1–3 Skilled Nursing Facilities or similar rehabilitation centers, 1–3 days per week at each location. Your primary focus will be delivering specialized physiatry care to 25+ patients per day who are experiencing difficulty participating in therapy due to pain or functional limitations.

Key responsibilities include:

• Performing focused assessments and developing individualized treatment plans in collaboration with facility primary care teams to optimize therapy participation and functional recovery.
• Providing targeted interventions such as joint injections, Botox treatments, dry needling, and medication management based on patient-specific needs.
• Serving as the sole physiatrist on-site, operating with a high level of autonomy while supported by a collaborative clinical team for consultation as needed.
• Coordinating care and referring to other specialties or services for issues beyond the scope of physiatry.

This role is best suited for a self-motivated and confident clinician who thrives in independent practice environments and is passionate about improving rehabilitation outcomes.

From $175,000.00+ per year in production compensation with compensation up to $260,000 possible.
