Job summary

Chestnut Health Company in Belleville, IL is seeking a highly motivated Nurse Practitioner to join our physiatry team. This role focuses on rehabilitation to restore function and independence for SNF patients, with emphasis on collaborative care and patient-centered outcomes.

Qualifications

Actively licensed to practice in Illinois (state of employment).
Current DEA license required on start date.
Current Controlled Substances license required on start date.
ANCC or AANP certification.
Interest in Physiatry/PM&R; SNF experience is a plus.

Responsibilities

Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive rehabilitation treatment plans.
Conduct patient assessments including histories, exams, and diagnostic evaluations to determine goals and interventions.
Monitor progress and adjust treatment plans to optimize functional outcomes and independence.
Participate in multidisciplinary team meetings and ongoing education to enhance clinical skills.
Maintain accurate documentation of evaluations, plans, progress notes, and discharge summaries in EMR systems.
Educate patients and families regarding rehabilitation process, expected outcomes, and post-discharge care.
