Job summary

Iconic Care Support Services Vallejo is seeking a Physiatry Nurse Practitioner Lead for regional oversight of clinical care. The role requires travel to facilities within the assigned region and rounds at your designated facility to aid patients with pain or functional limitations.

You will lead providers, coordinate treatment plans, and deliver interventions such as joint injections, ultrasound-guided procedures, and medication management in a highly autonomous setting.

Qualifications

5+ years as NP or PA.
Current, unrestricted NP or PA license.
DEA licensure or willing to obtain.
Skilled Nursing Facility experience.
Physiatry, Pain Management or PM&R experience.
Willingness to travel regularly within the assigned region.

Responsibilities

Regional oversight of clinical care and facility relationships.
Round 2-3 days per week at designated facility to care for patients.
Provide targeted interventions such as joint injections, Botox, dry needling, and meds management.
Coordinate care with primary care teams and refer to other specialties as needed.
Lead clinical training to ensure providers demonstrate strong decision making and bedside manner.
Maintain autonomy while supporting physiatrists on-site.

We’re Hiring: We are seeking a passionate, motivated provider to join us full-time and independently offer care and consultations at our contracted facilities.

Day-to-Day Responsibilities

Must Live in: San Francisco, CA; Antioch, CA; Tracy, CA; Oakland, CA or Sacramento, CA

Sacramento, Brentwood, San Leandro or Hayward areas preferred

This role would see either see facilities from San Jose to Oakland weekly, and facilities in Sacramento monthly or vice versa, depending on candidate home location, with compensated mileage.

Physician Assistants also welcome to apply.

Primarily responsible for regional oversight of clinical care and facility relationships, you will also round 2-3 days a week at your own designated facility to care for patients who are experiencing difficulty participating in therapy due to pain or functional limitations.

Key responsibilities include:

Clinical point of contact for all Providers in your region
Clinical training to ensure all Providers display excellent clinical decision making and bedside manner
Time shadowing each Provider in the region
Regular clinical and facility meetings with facility staff
Focused assessments and developing individualized treatment plans in collaboration with facility primary care teams to optimize therapy participation and functional recovery.
Providing targeted interventions such as joint injections, Botox treatments, dry needling, and medication management based on patient-specific needs.
Supporting physiatrists on-site and operating with a high level of autonomy
Coordinating care and referring to other specialties or services for issues beyond the scope of physiatry

This role is best suited for a self-motivated and confident clinician who thrives in independent practice environments and is passionate about improving rehabilitation outcomes.

Qualifications:

5+ years as a Nurse Practitioner (NP) or Physician Assistant (PA)
Current, unrestricted license as a Nurse Practitioner (NP) or Physician Assistant (PA)
DEA licensed or willing to obtain DEA licensure
Skilled Nursing Facility experience
Physiatry, Pain Management or Physical Medicine & Rehabilitation experience
Comfort in performing ultrasound guided peripheral joint injections (knees, shoulders, hips)
A genuine passion for providing professional, personalized care in an autonomous environment
Relationship and value proposition building with key stakeholders
Strong communication skills, with the ability to collaborate with multidisciplinary teams
Experience in a clinical leadership or supervisory role
Willingness to travel regularly within the assigned region
